Caring House Calls sees patients wherever they call home. We understand a person home is defined as where they reside. We provide care in Skilled Nursing Facilities, Assisted Living, or private residence. We can bridge the gap in care after a hospitalization, acute illness, or injury. We will see the patient earlier and reduce their chances for re-hospitalization. Caring House Calls coordinates all healthcare services with the patient’s primary care physician until the patient can return to scheduled office visits.
Telehealth
Telemedicine allows health care professionals to evaluate, diagnose and treat patients at a distance using telecommunications technology. The approach has been through a striking evolution in the last decade and it is becoming an increasingly important part of the American healthcare infrastructure.
In-Home Medical Care
Our NPs will assess the patient's needs, order medications and labs as medically necessary, and may send a summary of the visit to the patient’s primary care providers.
